The nursing process is a systematic framework in five stages and based on the problem solving approach; forms the basis of nursing practice to facilitate targeted and individualized care planning for patients (Yildirim and Ozkahraman 2011). This task will serve to identify the five phases of the nursing process: Assessment, Nursing Diagnosis, Planning, Implementation and Evaluation. The skills: communication, observation, critical thinking and reflection involved in the nursing process in collaboration with the patient will also be highlighted. The first phase of the nursing process is assessment. This is a continuous process from hospital admission to discharge. It involves gathering objective and subjective information about patients, through communication, observation, clinical knowledge and interpretation skills for decision making (Baath 2011). Objective data is collected from previous medical records, physical examinations, and laboratory tests, while subjective data represents the client's opinions about their health status (Corkin and Cardwell 2011). This information provides a complete understanding of the patient's health status. It also develops the basis for care planning and forms the remainder of the entire process, making it a crucial step (Nazarko 2011). Assessment consists of a range of skills, such as communication, observation and critical thinking. Communication is an essential skill for nursing, particularly in relation to the multidisciplinary team and the relationship with the patient. The quality of communication determines the outcome of treatment, the patient's well-being and the effectiveness of therapeutic partnerships (Boscart 2009).
