This project used a team of four to help manage veterans' care on a more intimate level, such as phoning their homes to find out how they are doing day to day day health is maintained and whether they have any current needs that the health care provider can help them meet. After reading their case study, I believe that having a healthcare team for each individual within a practice can also help address today's healthcare concerns. According to Porter and Lee (2013), this type of care would be called an integrated practice unit (IPU). In an IPU, a dedicated team made up of both clinical and non-clinical staff provides the entire course of care for the patient's condition. PUIs treat not only a disease but also the related conditions, complications, and circumstances that commonly occur along with it. IPUs not only provide care, but also take responsibility for engaging patients and their families in care by providing education and counseling, encouraging adherence to treatment and prevention protocols, and supporting necessary behavioral change (p.11). By using this type of team, better care can be provided to patients. This group would be composed of a doctor who will be the primary care physician and head of the group.
